Acknowledgement waivers, screening questionnaires NSSRC
Before entering the North Simcoe
Sports and Recreation Centre, all users must complete the
Acknowledgement, Release, Indemnity, and Assumption of Risk Regarding
COVID-19 waiver and the
NSSRC COVID-19 Screening Questionnaire, and
review the
Rules and Restrictions document. The questionnaire must be completed every time a participant wants to
enter the facility for training sessions.
Failure to do so will result in refusal of entry into the building.
