Acknowledgement waivers, screening questionnaires NSSRC
Submitted By Christine van der Weg on Friday, September 11, 2020
Before entering the North Simcoe Sports and Recreation Centre, all users must complete the Acknowledgement, Release, Indemnity, and Assumption of Risk Regarding COVID-19 waiver and the NSSRC COVID-19 Screening Questionnaire, and review the Rules and Restrictions document. The questionnaire must be completed every time a participant wants to enter the facility for training sessions.
Failure to do so will result in refusal of entry into the building.
